MEMBERSHIP FORM

To become a Member of the Burnaby Outdoor Club, you must complete and submit two forms: Once completed, forms can be scanned and submitted by email OR submitted by regular mail (instructions for mailing are included on the BOC Membership form).

Adults: FMCBC Universal Waiver Basic Form

FMCBC Acknowledgment of Risk for Minors (Note: If you are 16 or 17 years of age, you must complete the FMCBC form for Minors)
